• Father’s birth place
• Mother’s name
• Mother’s birth place
• County

The images of the original certificates will often be able to provide you with additional information about your ancestor:

• How long the deceased had been in Arizona
• Spouse’s name
• Whether any time was spent in a hospital or institution
• If the deceased was a veteran
• Exact age
• Occupation
• Place and date of burial, cremation, or removal
• Time of death
• Immediate cause of death

There are over 393,000 records in this collection of death certificates from Arizona. These certificates cover from the late 1800s to 1965.

Fourteen of the current fifteen counties of Arizona are included in these records. La Paz County, established in 1983, is not included as records only date to 1965. Counties included are as follows:
